Is the gold metallic or printed?

It is printed rather than foiled, so it reads as gold through color and shading rather than by reflecting light. That is the right choice for a shower, since foil finishes crack and lift in a wet room. Under a warm bulb the effect is convincing.

Does the paisley background compete with the skull?

Deliberately, but at a lower contrast. The ground is dark and the skull is bright, so from a distance you see the skull alone; close up the paisley resolves into separate medallions. It is a layered composition rather than two patterns fighting.

Is this too much for a small bathroom?

It depends entirely on the tile. In a small dark-tiled room it works, because the panel becomes the only light source in the composition. In a small white bathroom it will dominate everything else you own.

Do the colors survive repeated washing?

At 86 F yes. Digital prints on polyester hold saturation well; what damages them is heat and abrasion, so the dryer and a load full of towels are the two things to avoid.
