What carries these simple shower curtains is the hand of the cloth: a linen-look polyester in a washed denim blue, woven with enough slub in it that the surface catches light unevenly the way a real chambray does. Fine columns of small white dots run vertically through the blue at wide intervals, drawn like hand-stitched running thread rather than printed as a hard line. That is the entire pattern, and it is deliberately understated. The specially shaped bottom edge holds the drape even so the vertical columns stay upright rather than wandering at the hem. Held close, the blue is not even: paler threads run through the darker ones, which is what gives a washed rather than a dyed-flat appearance.

No. It is a polyester woven to look and handle like linen, with a slub texture and a dry matte surface. We describe it as linen-look for that reason. It behaves better than linen would in a wet room, since it does not hold water.
They are printed, drawn to resemble a line of hand running stitch rather than a mechanical dotted rule. Close up you can see they sit on the surface. At normal distance they read as stitching.
No, rings are not included and are sold separately. The header takes any standard hook or ring, so whatever is already on your rod will fit. Check before ordering if you are replacing a grommet-and-hook set.
